In conjunction with the American Heart Association, Check. Change. Control. uses self-monitoring and tracking of blood pressure readings at home to help you achieve and maintain a healthy heart.


Sign up for the Check, Change, Control (CCC) tracker through the American Heart Association (click here to sign up), to help record your blood pressure.


Furthermore, for closer monitoring, it is best to document blood pressure readings 2-3 times per week.
